Lifeline Documents Needed: proof checklist
Get approved faster by having the right proof ready.
Quick answer
What documents do I need to apply for Lifeline?
Most applicants need only their name, address, date of birth and the last four digits of their Social Security number. If the National Verifier cannot confirm you automatically, you upload a government ID plus one proof of eligibility, such as a SNAP or Medicaid letter, an SSI award letter or proof of income.
- ID
- Program or income proof
- Dated within 12 months
What documents do I need for a Free Government Phone?
You need two things: proof of identity and proof of eligibility. Most people only need their information, because the National Verifier confirms it electronically. Keep these ready just in case.
Proof of identity
- Driver's license or state ID
- U.S. passport or passport card
- Tribal ID
- Military ID
- Permanent resident card or other government-issued ID
Proof of program participation
| Program | Accepted proof |
|---|---|
| SNAP | SNAP approval or benefit letter, renewal notice |
| Medicaid | Medicaid letter or member card showing the program |
| SSI | SSA award or benefit verification letter |
| Public Housing | Housing authority notice or lease |
| Veterans Pension | VA award or benefit summary letter |
| Tribal programs | Letter from the Tribal program |
Proof of income
- Prior year tax return
- Three consecutive months of pay stubs
- Social Security or pension statement
- Unemployment or workers' compensation statement
- Child support or alimony award
What makes a document acceptable?
- Shows your name (or the household member who qualifies)
- Shows the program name or income amount
- Shows the issuing agency
- Is dated within the last 12 months (or is the most recent tax return)
- Is clear and readable, with all four corners
How do I upload documents?
If the verifier asks for proof, take a clear photo with your phone and upload it in the application. You can cover account numbers that are not needed. Our team in Tulsa can help at (918) 931-0991.
Common reasons documents are rejected
- The photo is blurry, cut off or too dark
- The document is older than 12 months
- The name does not match the application
- The document does not show the program name or the issuing agency
Retake the photo in good light on a flat surface, with all four corners showing.
Can I black out information on my documents?
Yes. You can cover account numbers, case numbers and other details that are not needed, as long as your name, the program or income amount, the agency and the date are still readable.
Still unsure which document to use? Call (918) 931-0991 and tell us which benefit you receive. We will tell you exactly what to upload.
